Description

  • Carvedilol is a medication used for the treatment of mild, moderate or severe heart failure of ischemic or cardiomyopathic origin. It is also used in the treatment of hypertension, angina pectoris, and congestive heart failure.

Indications

  • Treatment of mild, moderate or severe heart failure of ischemic or cardiomyopathic origin in conjunction with digitalis, diuretics, and ACE inhibitor
  • Reduction of progression of disease as evidenced by cardiovascular death, cardiovascular hospitalization, or the need to adjust other heart failure medications
  • Use in patients unable to tolerate an ACE inhibitor or not receiving digitalis, hydralazine or nitrate therapy

Pharmacology

  • Non-selective antagonism of β-adrenergic receptors
  • Appreciable a-adrenergic antagonistic activity
  • Antiproliferative properties
  • Scavenger of reactive free oxidant radicals

Dosage and Administration

  • Initial dosage for hypertension: 12.5 mg once daily, increased to 25 mg once daily with a maximum of 50 mg daily
  • Initial dosage for angina pectoris: 12.5 mg twice daily for the first 2 days, followed by 25 mg twice daily with a maximum of 50 mg daily for elderly patients
  • Initial dosage for heart failure: 3.125 mg twice daily, increasing at intervals to a maximum of 25 mg twice daily for patients weighing less than 85 kg and 50 mg twice daily for patients over 85 kg

Interaction

  • Increased monitoring of digoxin when initiating, adjusting, or discontinuing the dose of Cardex
  • Significant decrease in Cmax and AUC with pretreatment with rifampin
  • No alteration in the in vitro plasma protein binding of warfarin by Cardex
  • β-receptor antagonists potentiate the pressor reaction which may follow the sudden withdrawal of treatment with clonidine

Contraindications

  • Decompensated heart failure requiring intravenous inotropic therapy
  • Bronchial asthma or related bronchospastic conditions
  • Second or third-degree AV block
  • Sick sinus syndrome without a permanent pacemaker
  • Cardiogenic shock or severe bradycardia

Pregnancy and Lactation

Precautions and Warnings

  • Caution in hepatic impairment
  • Monitoring clinical status in heart failure after initiation and increasing each dose
