Indications
- Adjunct in treatment of parkinsonism

Description
- Exerts direct inhibitory effect on parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for M1 subtype
- Higher affinity for central muscarinic receptors in cerebral cortex
- Lower affinity for peripherally located receptors
- Possible modification of nicotinic acetylcholine receptor neurotransmission
- Enhanced dopamine release in the striatum
Dosage & Administration
- Initial low dose, gradually increased
- Individualized dosage
- Preferable administration before or after meals based on patient's reaction
- Potential need for small amounts of atropine in postencephalitic patients
- Precautions for dry mouth
- Abrupt withdrawal may result in exacerbation of symptoms or neuroleptic malignant syndrome
- Different dosages for idiopathic parkinsonism, drug-induced parkinsonism, concomitant use with levodopa, and children
- Best tolerated when divided into 3-4 doses at mealtimes
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensification of anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk of tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics
- Possible need for reduction of levodopa dosage during concomitant therapy

- Hypersensitivity to trihexyphenidyl
- Narrow angle glaucoma

Pregnancy & Lactation
- Use only if benefit outweighs risk during pregnancy

Precautions & Warnings
- Gonioscope evaluation prior to therapy
- Monitoring of intraocular pressures
- Possibility of angle closure with increase in intraocular pressure
- Administration caution in hot weather and concomitantly with other atropine-like drugs
- Anhidrosis management in hot environments
- Neuroleptic malignant syndrome risk with dose reduction or discontinuation
- Hyperthermia consideration in presence of anhidrosis
Overdose Effects
- Symptoms of atropine intoxication
- Neuropsychiatric signs
- Potential progression to stupor, coma, paralysis, cardiac and respiratory arrest, and death
- Gastric lavage or methods to limit absorption
- Administration of diazepam or barbiturate if CNS excitation observed
- Contraindicated phenothiazines
- Possible need for respiratory support, artificial respiration, or vasopressor agents
