Description
- Exerts a direct inhibitory effect upon the parasympathetic nervous system
- Has a relaxing effect on smooth musculature
- Indicated as an adjunct in the treatment of all forms of parkinsonism
Indications
- Useful as adjuvant therapy when treating parkinsonism with levodopa
- Control of extrapyramidal disorders caused by central nervous system drugs
Dosage & Administration
- Initial dose should be low and then increased gradually
- Dosage should be individualized
- Higher doses may be divided into 4 parts, with 3 doses administered at mealtimes and the fourth at bedtime
- Careful adjustment is necessary when used concomitantly with levodopa
Mode of Action
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to the M1 subtype
- Higher affinity for central muscarinic receptors located in the cerebral cortex
Interactions
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects when used with alcohol or other CNS depressants
- Potential risk of increased drug-induced involuntary movements during concomitant administration with certain medications
Contraindications
- Hypersensitivity to Trihexyphenidyl
- Patients with narrow angle glaucoma
Side Effects
- Dryness of the mouth
- Blurred vision
- Dizziness
- Mild nausea or nervousness
- Suppression of lactation
- Exacerbation of parkinsonism with abrupt treatment withdrawal
Pregnancy & Lactation
- Not recommended during pregnancy unless need has been clearly established
Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Caution in hot weather, especially when given concomitantly with other atropine-like drugs
- Possible occurrence of hyperthermia and fatal hyperthermia
Overdose Effects
- Central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, confusion, seizures may be present
- Treatment involves symptomatic and supportive therapy, gastric lavage, and respiratory support
