Indications

  • Adjunct in the treatment of parkinsonism
  • Adjuvant therapy with levodopa
  • Control of extrapyramidal disorders
  • Treatment of postencephalitic parkinsonism
  • Treatment of arteriosclerotic parkinsonism
  • Treatment of idiopathic parkinsonism

Description

  • Exerts inhibitory effect on parasympathetic nervous system
  • Relaxing effect on smooth musculature

Pharmacology

  • Non-selective muscarinic acetylcholine receptor antagonist
  • Binds with higher affinity to M1 subtype of receptors
  • Higher affinity for central muscarinic receptors in cerebral cortex
  • May modify nicotinic acetylcholine receptor neurotransmission
  • May lead to enhanced dopamine release in striatum

Dosage & Administration

  • Initial low dose, then increased gradually
  • Individualized dosage
  • Dosing based on patient's age and reaction
  • Can be taken before or after meals
  • Recommended dosages for different forms of parkinsonism
  • Concomitant use with levodopa dosage adjustments

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, and alcohol
  • Increased sedative effects with CNS depressants
  • Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
  • Possible risk of tardive dyskinesia with neuroleptics
  • Levodopa dose adjustments during concomitant therapy

Contraindications

  • Hypersensitivity to Trihexyphenidyl
  • Narrow angle glaucoma

  • Angle-closure glaucoma

Pregnancy & Lactation

  • Not enough data in animal or human pregnancy
  • Caution in nursing women due to potential excretion in human milk
  • Possible suppression of lactation

Precautions & Warnings

  • Gonioscope evaluation prior to therapy
  • Monitoring of intraocular pressures
  • Possibility of narrow angle glaucoma
  • Administration caution in hot weather
  • Potential for hyperthermia in patients with anhidrosis
  • Risk of Neuroleptic Malignant Syndrome

Overdose Effects

  • Central symptoms of atropine intoxication
  • Dilated and sluggish pupils
  • Warm, dry skin
  • Tachycardia
  • Neuropsychiatric signs
  • Stupor, coma, paralysis, cardiac and respiratory arrest
  • Gastric lavage for overdose treatment
  • Respiratory support and artificial respiration may be necessary
