Indications
- Adjunct in the treatment of all forms of parkinsonism
- Adjuvant therapy with levodopa for parkinsonism
- Control of extrapyramidal disorders caused by CNS drugs
Description
- Exerts direct inhibitory effect upon the parasympathetic nervous system
- Relaxing effect on smooth musculature
- Inhibitory effect upon parasympathetic nervous system
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for central muscarinic receptors in cerebral cortex
- Lower affinity for peripherally located receptors
- Modification of nicotinic acetylcholine receptor neurotransmission
- Enhanced dopamine release in the striatum
Dosage & Administration
- Individualized dosage
- Low initial dose with gradual increase
- Dose adjustment based on patient reaction
- Preferable timing in relation to meals
- Caution for patients over 60 years of age
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensified anticholinergic effects with MAO inhibitors and tricyclic antidepressants
- Risks of drug-induced involuntary movements with concomitant use of anticholinergics and levodopa

- Hypersensitivity to trihexyphenidyl
- Narrow angle glaucoma

- Caution in hot weather and concomitant use with other atropine-like drugs
- Risk of anhidrosis and hyperthermia
- Potential for Neuroleptic Malignant Syndrome (NMS)
