Indications

  • Adjunct in the treatment of all forms of parkinsonism
  • Adjuvant therapy with levodopa for parkinsonism
  • Control of extrapyramidal disorders caused by CNS drugs

Description

  • Exerts direct inhibitory effect upon the parasympathetic nervous system
  • Relaxing effect on smooth musculature
  • Inhibitory effect upon parasympathetic nervous system

Pharmacology

  • Non-selective muscarinic acetylcholine receptor antagonist
  • Higher affinity for central muscarinic receptors in cerebral cortex
  • Lower affinity for peripherally located receptors
  • Modification of nicotinic acetylcholine receptor neurotransmission
  • Enhanced dopamine release in the striatum

Dosage & Administration

  • Individualized dosage
  • Low initial dose with gradual increase
  • Dose adjustment based on patient reaction
  • Preferable timing in relation to meals
  • Caution for patients over 60 years of age

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, and alcohol
  • Increased sedative effects with alcohol or other CNS depressants
  • Intensified anticholinergic effects with MAO inhibitors and tricyclic antidepressants
  • Risks of drug-induced involuntary movements with concomitant use of anticholinergics and levodopa

Contraindications

  • Hypersensitivity to trihexyphenidyl
  • Narrow angle glaucoma

Precautions & Warnings

  • Gonioscope evaluation prior to therapy
  • Monitoring of intraocular pressures
  • Potential for angle closure and increase in intraocular pressure
  • Caution in hot weather and concomitant use with other atropine-like drugs
  • Risk of anhidrosis and hyperthermia
  • Potential for Neuroleptic Malignant Syndrome (NMS)

Overdose Effects
