Indications
- Adjunct in the treatment of all forms of parkinsonism
- Useful as adjuvant therapy with levodopa
- Control of extrapyramidal disorders caused by CNS drugs
Description
- Exerts direct inhibitory effect upon the parasympathetic nervous system
- Has a relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to the M1 subtype
Dosage & Administration
- Initial dose should be low and increased gradually, especially in patients over 60 years of age
- Dosage should be individualized
- Total daily dose depends upon optimal level
- Careful adjustment of dosage form, amount of drug, or interval between doses can control minor side effects
Interaction
- Cannabinoids, barbiturates, opiates, alcohol may have additive effects
- Concurrent use of alcohol or other CNS depressants may cause increased sedative effects
- Monoamine oxidase inhibitors and tricyclic antidepressants with significant anticholinergic activity may intensify anticholinergic effects
Contraindications
- Contra-indicated in patients with hypersensitivity to Trihexyphenidyl
- Contra-indicated in patients with narrow angle glaucoma

- Dryness of the mouth

- Nausea
- Nervousness
- Paralytic ileus
- Psychiatric manifestations such as delusions, hallucinations, paranoia
- Increased intraocular pressure
- Weakness
- Vomiting
- Headache
Pregnancy & Lactation
- Not recommended during pregnancy unless need clearly established and benefit outweighs risk
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Patients should have a gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures should be conducted
- Use with caution in hot weather and with other atropine-like drugs
- Anhidrosis may occur more readily when some disturbance of sweating already exists
Overdose Effects
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, disorientation, seizures may be present
- Treatment involves symptomatic and supportive therapy
