Indications

  • Hypertension
  • Reduce hospitalizations in patients with congestive heart failure
  • Reduce death in patients who developed congestive heart failure after myocardial infarction

Pharmacology

  • Oral medication
  • Belongs to class of drugs called angiotensin receptor blockers (ARBs)
  • Blocks angiotensin II receptor
  • Dilates blood vessels
  • Reduces blood pressure

Dosage

  • Usual dosage of 80-160 mg once daily for hypertension
  • 40 mg twice daily for heart failure
  • 20 mg twice daily for post-myocardial infarction
  • Maximum dose of 320 mg daily for hypertension

Administration

  • Should be taken on an empty stomach
  • No initial dosage adjustment for elderly patients with mild to moderate renal and hepatic insufficiency

Interaction

  • No clinically significant drug interactions found
  • Usually not metabolized to a significant extent
  • May lead to increase in serum potassium if used with potassium sparing diuretics or supplements

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product

  • Rare side effects
  • Including headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea, hyperkalemia, impotency, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo, hypotension

Pregnancy & Lactation

  • Should not be used in pregnancy due to potential harm to fetus
  • Not known whether excreted in human milk

Precautions & Warnings

  • Care should be exercised in patients with impaired hepatic function
  • Dosage reduction or discontinuation may be required with pre-existing renal impairment
  • Caution should be exercised when initiating therapy in patients with heart failure and post-myocardial infarction patients

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • No overall difference in efficacy or safety observed in geriatric patients
  • Dosage adjustment in patients with hepatic impairment

Overdose Effects

  • Potential manifestations of hypotension and tachycardia
  • Supine position and intravenous infusion of normal saline may be required
