Indications
- Hypertension
- Congestive heart failure
- Reduction of hospitalizations
- Reduction of death after myocardial infarction
Pharmacology
- Belongs to class of drugs called angiotensin receptor blockers (ARBs)
- Orally active
- Acts on the AT1 subtype
- Blocks angiotensin II receptor
- Dilates blood vessels
- Reduces blood pressure without affecting pulse rate
Dosage
- 80 to 160 mg once daily for hypertension
- 40 mg twice daily for heart failure
- 20 mg twice daily for post-myocardial infarction
- Maximum dose is 320 mg daily
Administration
- To be taken on an empty stomach
- Decreases absorption by about 40% when taken with food
- No initial dosage adjustment required for elderly patients with mild to moderate renal and hepatic insufficiency
Interaction
- No drug interactions of clinical significance found
- Compounds studied in clinical trials include Cimetidine, Warfarin, Furosemide, Digoxin, Atenolol, Indomethacin, Hydrochlorothiazide, Amlodipine, and Glibenclamide

- Exercise care in patients with mild to moderate hepatic impairment
- Dosage reduction may be required in patients with pre-existing renal impairment

Use in Special Populations
- Safety and effectiveness in pediatric patients have not been established
- No overall difference in the efficacy or safety in geriatric patients
- Caution in patients with hepatic impairment
Overdose Effects
- Likely manifestations include hypotension and tachycardia
- Bradycardia could occur from parasympathetic (vagal) stimulation
- Treatment may involve placing the patient in the supine position and intravenous infusion of normal saline
