Indications
- used for hypertension
- reduces hospitalizations in patients with congestive heart failure
- reduces death in patients who developed congestive heart failure after myocardial infarction
Pharmacology
- oral medication
- angiotensin receptor blockers (ARBs)
- specific angiotensin II antagonist
- blocks the angiotensin II receptor
- dilates blood vessels
- reduces blood pressure without affecting pulse rate
Dosage
- usual dose range: 80 to 320 mg for hypertension
- usual dose range: 40 to 160 mg twice daily for heart failure
- initial dose: 20 mg twice daily for post-myocardial infarction
Administration
- should be taken on an empty stomach
- decreases absorption by about 40%
Interaction
- no major drug interactions
- no clinically relevant metabolic interactions
- caution advised with potassium sparing diuretics or supplements
Contraindications
- contraindicated in patients hypersensitive to any component
Side Effects
- rare side effects
- common side effects include headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea
- may cause hyperkalemia, impotency, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo
- hypotension may occur when taken with diuretics
Pregnancy & Lactation
- should not be used in pregnancy
- potential adverse effects on nursing infants
Precautions & Warnings
- exercise caution in patients with impaired hepatic function
- dosage reduction may be required with pre-existing renal impairment
- caution in patients with heart failure and post-myocardial infarction
Use in Special Populations
- safety and effectiveness not established in pediatric patients
- no overall difference in efficacy or safety observed in geriatric patients
- dosage adjustment in mild to moderate hepatic impairment
- severe hepatic impairment is contraindicated
Overdose Effects
- likely manifestations include hypotension and tachycardia
- bradycardia could occur from parasympathetic stimulation
- intravenous infusion of normal saline may be necessary
