Indications

  • Treatment of hypertension

Pharmacology

  • Belongs to a class of drugs called angiotensin receptor blockers (ARBs)
  • Specific angiotensin II antagonist
  • Orally active
  • Blocks angiotensin II receptor
  • Thiazide diuretic
  • Affects renal tubular mechanisms of electrolyte reabsorption
  • Reduces plasma volume
  • Increases plasma renin activity
  • Increases aldosterone secretion
  • Increases urinary potassium loss
  • Decreases serum potassium

Dosage & Administration

  • Once daily dosage
  • Maximum allowed dose of Valsartan in combination with hydrochlorothiazide
  • No initial dosage adjustment for elderly patients
  • Safety and effectiveness in pediatric patients not established

Interaction

  • May interact with diuretics, agents increasing serum potassium, lithium salts, warfarin, digoxin, alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, cholestyramine and colestipol resins

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product
  • Contraindicated in patients with anuria
  • Contraindicated in patients hypersensitive to sulfonamide-derived drugs

Side Effects

  • Rare side effects include headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea, hyperkalemia, impotence, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo
  • Hypotension may occur

Pregnancy & Lactation

  • Not to be used in pregnancy
  • Not known whether excreted in human milk

Precautions & Warnings

  • Exercise care in patients with impaired hepatic function
  • Dosage reduction or discontinuation may be required if pre-existing renal impairment
  • Caution in initiating therapy in patients with heart failure and post-myocardial infarction patients

Use in Special Populations

  • Usual regimens of therapy may be followed as long as the patient's creatinine clearance is >30 ml/min
  • Loop diuretics preferred in patients with severe renal impairment
  • Care should be taken in patients with hepatic impairment

Overdose Effects

  • Manifestations of overdosage include hypotension, tachycardia, bradycardia, hypokalemia, hypochloremia, dehydration
  • Excessive diuresis can lead to electrolyte depletion
  • Hemodialysis removal not established
