Indications

  • Treatment of hypertension

Pharmacology

  • Belongs to class of drugs called angiotensin receptor blockers (ARBs)
  • Blocks angiotensin II receptors
  • Thiazide diuretic affecting renal tubular mechanisms

Dosage & Administration

  • Combination therapy for uncontrolled blood pressure
  • Maximum dose of Valsartan and Hydrochlorothiazide combination
  • No initial dosage adjustment for elderly patients
  • Not established for pediatric use

Interaction

  • Potential interaction with diuretics
  • Affects serum potassium levels
  • Interaction with lithium salts, Warfarin, and Digoxin
  • Interactions with alcohol, barbiturates, narcotics, antidiabetic drugs, and other antihypertensive drugs
  • Impaired absorption in presence of anionic exchange resins

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria
  • Hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Rare side effects
  • Common side effects: headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea
  • Potential side effects: hyperkalemia, impotency, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo, hypotension

Pregnancy & Lactation

  • Not to be used in pregnancy
  • Excretion in breast milk

Precautions & Warnings

  • Care with impaired hepatic function
  • Dosage adjustment or discontinuation with pre-existing renal impairment
  • Caution in heart failure and myocardial infarction patients

Overdose Effects

  • Manifestations of Valsartan overdosage: hypotension, tachycardia, bradycardia, parasympathetic stimulation
  • Management of Valsartan overdosage: supine positioning, intravenous infusion of normal saline
  • Manifestations of Hydrochlorothiazide overdosage: electrolyte depletion, dehydration
  • Potential interaction with digitalis
  • Unknown removal by hemodialysis
