Indications
- Treatment of mild, moderate or severe heart failure
- Treatment of heart failure of ischemic or cardiomyopathic origin
- Reduction of disease progression
- Conjunction with digitalis, diuretics and ACE inhibitor
- Use in patients unable to tolerate an ACE inhibitor
- Use in patients not receiving digitalis, hydralazine or nitrate therapy

Dosage & Administration
- Initial dosage for hypertension: 12.5 mg once daily
- Usual dosage for hypertension: 25 mg once daily
- Maximum dosage for hypertension: 50 mg daily
- Initial dosage for angina pectoris: 12.5 mg twice daily
- Usual dosage for angina pectoris: 25 mg twice daily
- Maximum dosage for angina pectoris: 50 mg daily
- Initial dosage for heart failure: 3.125 mg twice daily
- Usual dosage for heart failure: 25 mg twice daily
- Maximum dosage for heart failure: 25 mg twice daily (body-weight <85 kg) or 50 mg twice daily (body-weight >85 kg)

- Interaction with digoxin
- Interaction with rifampin
- Interaction with warfarin
- Interaction with clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock
- Severe bradycardia

Pregnancy & Lactation
- Not to be used during breast-feeding
- Animal studies showing excretion in breast milk

Precautions & Warnings
- Caution in hepatic impairment
- Monitor clinical status after initiation and increasing each dose
- Ensure renal function and heart failure are not deteriorating
