Dosage and Administration
- Initial Dose: 12.5 mg once daily for hypertension
- Increased to 25 mg once daily for hypertension
- Maximum Dose: 50 mg daily for hypertension
- Initial Dose: 12.5 mg twice daily for angina pectoris
- Increased to 25 mg twice daily for angina pectoris
- Maximum Dose: 50 mg daily for angina pectoris
- Initial Dose: 3.125 mg twice daily for heart failure
- Dose increases at intervals of at least 2 weeks
- Maximum Dose: 25 mg twice daily for patients less than 85 kg body-weight
- Maximum Dose: 50 mg twice daily for patients over 85 kg
Indications
- Treatment for mild, moderate, or severe heart failure
- Treatment for ischemic or cardiomyopathic origin heart failure
- Reduction of disease progression
- Reduction of cardiovascular death
- Reduction of cardiovascular hospitalization

- Non-selective antagonism of beta-adrenergic receptors
- Alpha-adrenergic antagonistic activity
- Antihypertensive properties
- Antianginal properties
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
Interactions
- Increased levels of digoxin when taken together
- Decreased Cmax and AUC when pretreated with rifampin
- No alteration of plasma protein binding of warfarin
- Potentiation of pressor reaction with clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock
- Severe bradycardia
