Indications

Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- incretin hormones
- glucagon-like peptide-1 (GLP-1)
- glucose-dependent insulinotropic polypeptide (GIP)
- physiologic regulation of glucose homeostasis
Dosage & Administration
- 50 mg daily for monotherapy
- 50 mg twice daily for combination with Metformin or Thiazolidinedione
- 50 mg once daily in the morning for combination with a Sulphonylurea
- may be taken with or without a meal
- no dosage adjustment needed for elderly or mild renal impairment
- not recommended in patients under 18 years
Interaction
- no interactions with pioglitazone, metformin, glibenclamide, digoxin, warfarin, amlodipine, ramipril, valsartan, or simvastatin
- glucose-lowering effect may be reduced by thiazides, corticosteroids, thyroid products, sympathomimetics
Contraindications
- Hypersensitivity to the active substance or excipients
- moderate to severe renal impairment
- pre-treatment alanine aminotransferase (ALT) or aspartate aminotransferase (AST) >3 times the upper limit of normal (ULN)
- type 1 diabetes
Side Effects
- majority of adverse reactions mild and transient
- rare case of hepatic dysfunction
- long-term trials did not show additional safety signals
Pregnancy & Lactation
- should not be used in pregnancy
- should not be used during lactation
Precautions & Warnings
- caution in patients aged 75 years and older
- monitor liver function tests (LFTs)
- discontinue treatment if transaminase levels increased
- do not reinitiate treatment after withdrawal
- use with caution in patients with congestive heart failure
- do not use in patients with NYHA functional class III IV
