Indications
- Adjunct to diet and exercise
- Improves glycemic control
- Type 2 diabetes mellitus
- Monotherapy
- Dual combination with Metformin, Sulphonylurea, Thiazolidinedione, or Insulin
Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Slows inactivation of incretin hormones
- Increases insulin release
- Decreases glucagon levels
- Glucose-dependent manner
Dosage & Administration
- Recommended doses: 50mg or 100mg daily for monotherapy
- 50mg twice daily for dual combination with Metformin or Thiazolidinedione
- 50mg once daily for dual combination with Sulphonylurea
- May be taken with or without a meal
- No dosage adjustment required in the elderly or patients with mild renal impairment
Interaction
- No interactions with several medications observed
- Glucose-lowering effect may be reduced by certain active substances such as thiazides, corticosteroids, thyroid products, and sympathomimetics
Contraindications
- Hypersensitivity to the active substance or excipients
- Moderate to severe renal impairment
- Hepatic impairment
- Type 1 diabetes
Side Effects
- Majority of adverse reactions are mild and transient
- Rare case of hepatic dysfunction observed
- No unforeseen risks in long-term use
Pregnancy & Lactation
- Not to be used in pregnancy or lactation
Precautions & Warnings
- Caution in patients aged 75 years and older
- Monitor LFTs prior to initiation and periodically thereafter
- Discontinue if transaminase levels are increased
- Do not reinitiate treatment following withdrawal
- Use with caution in patients with congestive heart failure
- Do not use in patients with NYHA functional class III IV
