Indications

  • Adjunct to diet and exercises
  • Improves glycaemic control
  • Type 2 diabetes mellitus

Pharmacology

  • Inhibition of DPP-4 enzyme
  • Increase in incretin hormones GLP-1 and GIP
  • Insulin secretion from pancreatic beta cell
  • Decrease in glucagon secretion from alpha cell
  • Decreased hepatic glucose production

Dosage & Administration

  • Initiation based on current Metformin dose
  • Twice daily administration
  • May be taken with food
  • No clinical experience in triple combination with other antidiabetic agents

Interaction

  • No clinically relevant pharmacokinetic interaction observed
  • Low potential for drug interactions
  • Not likely to interact with CYP 450 enzymes
  • No clinically relevant interactions with other oral antidiabetics
  • Effects of co-administration with furosemide, nifedipine, and glyburide on Metformin

Contraindications

  • Known hypersensitivity to Vildagliptin or Metformin Hydrochloride
  • Renal disease or dysfunction
  • Acute myocardial infarction
  • Septicaemia
  • Congestive heart failure
  • Acute or chronic metabolic acidosis
  • Use during radiologic studies with intravascular administration

Side Effects

  • Headache
  • Tremor
  • Dizziness
  • Nausea
  • Hypoglycemia

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Not recommended during pregnancy unless potential benefit justifies the potential risk to the foetus
  • No studies conducted regarding excretion in human milk
  • Not recommended for breastfeeding women

Precautions & Warnings

  • Occurrence of Lactic acidosis due to Metformin accumulation
  • Monitoring of serum creatinine in patients
  • Special caution in elderly patients
  • Recommendation for Liver Function Tests (LFTs) monitoring
  • Caution in patients with jaundice or signs of liver dysfunction
  • Discontinuation before elective surgery with general anaesthesia

Use in Special Populations

  • Not recommended for pediatric patients
  • Monitoring of renal function in elderly patients
  • Use only in elderly patients with normal renal function
  • Not recommended in patients with renal or hepatic impairment
