Indications
- Adjunct to diet and exercises for glycaemic control in type 2 diabetes mellitus
- Improves insulin secretion
- Decreases glucagon secretion
- Reduces hepatic glucose production
Pharmacology
- Acts by inhibiting DPP-4 enzyme
- Increases fasting and postprandial endogenous levels of incretin hormones
- Lowers basal and postprandial plasma glucose
Dosage & Administration
- Initiated at twice daily
- May be taken with or just after food
- No clinical experience in triple combination with other antidiabetic agents
Interaction
- No clinically relevant interaction observed with 100mg Vildagliptin and 1000mg Metformin
- Low potential for drug interactions
- Does not likely interact with CYP 450 enzymes
- No clinically relevant interactions with other oral antidiabetics
- Furosemide, nifedipine and glyburide increase Cmax and blood AUC of Metformin
Contraindications
- Known hypersensitivity to Vildagliptin or Metformin Hydrochloride
- Renal disease or dysfunction
- Acute myocardial infarction and septicaemia
- Congestive heart failure patients
- Acute or chronic metabolic acidosis, including diabetic ketoacidosis
- Use of iodinated contrast materials
Side Effects
- Headache
- Tremor
- Dizziness
- Nausea
- Hypoglycemia
Pregnancy & Lactation
- Not recommended during pregnancy unless potential benefit justifies risk
- Not known if excreted in human milk
Precautions & Warnings
- Lactic acidosis can occur due to Metformin accumulation
- Serum creatinine should be monitored
- Special caution in elderly patients
- Liver Function Tests (LFTs) monitoring recommended
- Discontinue before elective surgery with general anaesthesia
Use in Special Populations
- Not recommended for use in children below 18 years of age
- Regular renal function monitoring in elderly patients
- Not recommended in patients with renal or hepatic impairment
