Indications
- Treatment of heart failure
- Reduction of disease progression
- Adjunct with digitalis, diuretics, and ACE inhibitor
- Use in patients unable to tolerate ACE inhibitor
- Use in patients not receiving digitalis, hydralazine or nitrate therapy

Dosage & Administration
- Initial dose for hypertension: 12.5 mg once daily
- Usual dose for hypertension: 25 mg once daily
- Initial dose for angina pectoris: 12.5 mg twice daily
- Usual dose for angina pectoris: 25 mg twice daily
- Initial dose for heart failure: 3.125 mg twice daily
- Usual dose for heart failure: 25 mg twice daily
- Maximum dosage for elderly patients: 50 mg daily
Interaction
- Increased monitoring of digoxin recommended
- Rifampin decreases Cmax and AUC
- No alteration of warfarin's plasma protein binding
- Potentiation of pressor reaction by clonidine withdrawal
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome (unless a permanent pacemaker is in place)
- Cardiogenic shock
- Severe bradycardia
