Indications

  • Adjunct to diet and exercises
  • Improves glycaemic control
  • Type 2 diabetes mellitus

Pharmacology

  • Inhibition of DPP-4 enzyme
  • Increase in insulin secretion
  • Decrease in glucagon secretion
  • Lowering of basal and postprandial plasma glucose

Dosage & Administration

  • Initiation based on current Metformin dose
  • Twice daily dosing
  • Gastrointestinal symptom reduction with food intake
  • No clinical experience in triple combination with other antidiabetic agents

Interaction

  • No clinically relevant pharmacokinetic interactions observed
  • Low potential for drug interactions
  • No clinically relevant interactions with other oral antidiabetics
  • Furosemide, nifedipine, and glyburide increase Cmax and blood AUC of Metformin

Contraindications

  • Known hypersensitivity to Vildagliptin or Metformin Hydrochloride or excipients
  • Renal disease or renal dysfunction
  • Acute myocardial infarction and septicaemia
  • Congestive heart failure
  • Acute or chronic metabolic acidosis
  • Radiologic studies involving intravascular administration of iodinated contrast materials

  • Headache
  • Tremor
  • Dizziness
  • Nausea
  • Hypoglycemia

Pregnancy & Lactation

  • Not recommended during pregnancy
  • No studies conducted in pregnant women
  • Not known whether excreted in human milk

Precautions & Warnings

  • Lactic acidosis risk
  • Serum creatinine monitoring
  • Caution in elderly patients
  • Liver function tests monitoring
  • Discontinue for jaundice or liver dysfunction
  • Discontinue before elective surgery with general anesthesia

Use in Special Populations

  • Not recommended for pediatric patients
  • Renal function monitoring in geriatric patients
  • Not for use in patients with renal or hepatic impairment
