Indications
- Treatment of Hodgkin’s disease
- Treatment of advanced testicular carcinoma
- Palliative treatment of lymphocytic lymphoma
- Treatment of choriocarcinoma
- Treatment of carcinoma of the breast
Pharmacology
- M phase specific
- Binds to microtubular proteins
- Arrests mitosis at metaphase
- Blocks glutamic acid utilization
- Inhibits purine synthesis
Dosage & Administration
- Initial adult dosage: 3.7 mg/m^2
- Usual adult dosage: 5.5-7.4 mg/m^2 per week
- Initial child dosage: 2.5 mg/m^2 of BSA
- Max weekly dose for adults: 18.5 mg/m^2
- Max weekly dose for children: 12.5 mg/m^2
- Duration of maintenance therapy depends on disease state and combination with other antineoplastic agents
Interaction
- Possible increase in vinblastine levels with aprepitant
- Reduced vinblastine metabolism with miconazole
- Variable interactions with phenytoin
- Additive myelotoxicity with zidovudine
- Concurrent admin of vinblastine with CYP3A inhibitors may cause earlier onset and increased severity of side effects
- Reduced immune response with vaccines
Contraindications
- Severe bone marrow suppression
- Presence of bacterial infection
- Malignant cell infiltration of bone marrow
- Inj into extremity with poor circulation
- Porphyria
- Granulocytopenia
- Elderly with cachexia or extreme skin ulcerations
- Pregnancy
- Lactation
- Intrathecal use

Use in Special Populations
- Hepatic Impairment: Reduce dose by 50% if serum bilirubin >3 mg/100ml
Overdose Effects
- Symptoms: Severe bone marrow suppression and extensions of usual side effects
- Management: Supportive treatment, monitor CV system and daily blood counts
