Indications
- Treatment of mild, moderate or severe heart failure
- Reduction of disease progression
- Use in patients unable to tolerate an ACE inhibitor
- Use in patients not receiving digitalis, hydralazine or nitrate therapy

Dosage & Administration
- Initial dose of 12.5 mg once daily for hypertension
- Initial dose of 12.5 mg twice daily for angina pectoris
- Initial dose of 3.125 mg twice daily for heart failure
- Maximum daily dose of 50 mg for angina pectoris in elderly patients
- Maximum doses for heart failure based on body-weight
Interaction
- Significantly increased levels of digoxin when taken together
- 60% decrease in Cmax and AUC with rifampin
- No alteration of in vitro plasma protein binding of warfarin
- Potentiation of the pressor reaction by clonidine withdrawal
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock or severe bradycardia

Precautions & Warnings
- Caution in hepatic impairment
- Monitor clinical status for 2-3 hours after initiation and dose increases
- Ensure renal function and heart failure are not deteriorating before increasing dose
