Indications
- Prophylaxis and/or treatment of thromboembolic complications associated with atrial fibrillation and/or cardiac valve replacement
- Reduction of the risk of death, recurrent myocardial infarction, and thromboembolic events such as stroke or systemic embolization after myocardial infarction
- Prophylaxis and treatment of venous thrombosis and pulmonary embolism
- Treatment of transient ischaemic attacks
Description
- Anticoagulant that inhibits the synthesis of vitamin K dependent clotting factors
- Acts by inhibiting the synthesis of Factors II, VII, IX, and X
- Anticoagulant effect occurs within 24 hours
- Maximum effect reached in 36-48 hours
- Effect is maintained for 48 hours or more after administration is stopped
Pharmacology
- Warfarin is a vitamin K antagonist
- Inhibits the production of vitamin K by vitamin K epoxide reductase
- Inhibition interrupts the coagulation cascade
- Results in a hypercoagulable state for a short time
- Vitamin K must be supplied to reverse the anticoagulation
Dosage & Administration
- Usual adult induction dose of warfarin is 10 mg daily for 2 days
- Subsequent maintenance dose is usually 3 to 9 mg daily
- Initial dose for children is usually 0.1 mg.kg-1.d-1
- Regular prothrombin time tests are essential
- Baseline prothrombin time should be determined before the initial dose
Interaction
- Oral anticoagulants have a greater potential for clinically significant drug interactions
Contraindications
- Not to be administered in actual or potential hemorrhagic conditions
- Severe hepatic or renal disease
- Pregnancy
- Known hypersensitivity to warfarin
- Bacterial endocarditis
Side Effects
- Principal adverse effect is hemorrhage
- Other adverse reactions include nausea, vomiting, diarrhea, hypersensitivity, rash, alopecia
- Unexplained drop in haematocrit, "purple toes", skin necrosis, jaundice, and hepatic dysfunction
Pregnancy & Lactation
- Contraindicated in the first trimester of pregnancy
- Should not be used in women who are or may become pregnant
- Passes through the placental barrier and may cause fatal hemorrhage to the fetus
- Appears in the milk of nursing mothers in an inactive form
Precautions & Warnings
- Periodic determination of prothrombin time (PT)/international normalized ratio (INR) is essential
- Numerous factors may influence response to anticoagulants, including travel, changes in diet, environment, physical state and medication
- Elderly subjects may exaggerate the effects of warfarin
- Careful additional laboratory control is necessary when changing from one formulation to another
Overdose Effects
- Excessive depression of the coagulation activity can be corrected by temporary withdrawal of warfarin
- Accompanied, if necessary, by infusion of fresh-frozen plasma or whole blood
- Vitamin K may be required to supplement specific treatment
