Indications

Dosage & Administration

  • Omitted maintenance dose if prothrombin time is excessively prolonged
  • Safety and efficacy for children <18 years old not established

Contraindications

  • Hemorrhagic conditions, uncontrolled hypertension, severe hepatic or renal disease, pregnancy, hypersensitivity to warfarin, bacterial endocarditis
  • Caution within 24 hours following surgery or labor
