Indications

Dosage & Administration
- Usual adult induction dose of warfarin: 10 mg daily for 2 days
- Subsequent maintenance dose depends on prothrombin time reported as INR
- Omitted maintenance dose if prothrombin time is excessively prolonged
- Safety and efficacy for children <18 years old not established
Contraindications
- Hemorrhagic conditions, uncontrolled hypertension, severe hepatic or renal disease, pregnancy, hypersensitivity to warfarin, bacterial endocarditis
- Caution within 24 hours following surgery or labor
