Indications

  • Prophylaxis and/or treatment of thromboembolic complications associated with atrial fibrillation and/or cardiac valve replacement
  • Reduction of risk of death, recurrent myocardial infarction, stroke, or systemic embolization after myocardial infarction
  • Prophylaxis and treatment of venous thrombosis and pulmonary embolism
  • Treatment of transient ischaemic attacks

Description

  • Anticoagulant that inhibits the synthesis of vitamin K dependent clotting factors
  • Acts by inhibiting the production of vitamin K by vitamin K epoxide reductase
  • Anticoagulant effect occurs within 24 hours after administration

Pharmacology

  • Acts as a vitamin K antagonist
  • Inhibits the production of vitamin K by vitamin K epoxide reductase
  • Inhibits the synthesis of vitamin K dependent clotting factors

DosageAdministration

  • Usual adult induction dose of warfarin is 10 mg daily for 2 days
  • Daily maintenance dose of warfarin is usually 3 to 9 mg
  • Maintenance dosage adjusted according to regular prothrombin time tests

Interaction

  • Oral anticoagulants have a greater potential for clinically significant drug interactions
  • Patients should be warned about potential hazards and instructed against taking or withdrawing any drug without the advice of a physician

Contraindications

  • Cannot be administered in cases of actual or potential hemorrhagic conditions
  • Not suitable for patients with severe hepatic or renal disease
  • Contraindicated in pregnancy and known hypersensitivity to warfarin
  • Not to be used within 24 hours following surgery or labor

SideEffects

  • Principal adverse effect is hemorrhage
  • Other adverse reactions include nausea, vomiting, diarrhea, hypersensitivity, rash, alopecia, unexplained drop in hematocrit, 'purple toes', skin necrosis, jaundice, and hepatic dysfunction

PregnancyLactation

  • Contraindicated in the first trimester of pregnancy due to risk of teratogenicity
  • Should not be used in women who are or may become pregnant as it may cause fatal hemorrhage to the fetus
  • Appears in the milk of nursing mothers in an inactive form

PrecautionsWarnings

  • Periodic determination of prothrombin time (PT)/international normalized ratio (INR) is essential
  • Response to anticoagulants may be influenced by travel, diet, environment, physical state, and medication
  • Good practice to monitor the patient's response with additional PT/INR determination after discharge from the hospital, and whenever other medications are initiated, discontinued, or taken irregularly

OverdoseEffects

  • Excessive depression of coagulation activity can be corrected by temporary withdrawal of warfarin
  • Vitamin K may be required to supplement specific treatment with co-factor concentrates
