Indications
- Prophylaxis and/or treatment of thromboembolic complications associated with atrial fibrillation and/or cardiac valve replacement
- Indicated to reduce the risk of death, recurrent myocardial infarction, and thromboembolic events such as stroke or systemic embolization after myocardial infarction
- Prophylaxis and treatment of venous thrombosis and pulmonary embolism
- Transient ischaemic attacks
Description
- Anticoagulant that inhibits the synthesis of vitamin K dependent clotting factors
- Acts on clotting factors II, VII, IX and X
- Anticoagulant effect occurs within 24 hours after administration
Pharmacology
- Acts as a vitamin K antagonist
- Inhibits the production of vitamin K by vitamin K epoxide reductase
- Affects coagulation factors VII, IX, X, and thrombin
Dosage & Administration
- Usual adult induction dose is 10 mg daily for 2 days
- Subsequent maintenance dose depends upon prothrombin time
- Maintenance dose for adults is usually 3 to 9 mg daily
Interaction
- Has greater potential for clinically significant drug interactions
- Patients should be warned against taking or withdrawing any drug without physician advice
Contraindications
- Cannot be administered in cases of actual or potential hemorrhagic conditions
- Severe hepatic or renal disease
- Pregnancy
- Known hypersensitivity to warfarin
- Bacterial endocarditis
Side Effects
- Principal adverse effect is hemorrhage
- Other adverse reactions include nausea, vomiting, and diarrhea
Pregnancy & Lactation
- Contraindicated in the first trimester of pregnancy
- Passes through the placental barrier and may cause fatal hemorrhage to the fetus
- Appears in the milk of nursing mothers in an inactive form
Precautions & Warnings
- Periodic determination of prothrombin time (PT)/international normalized ratio (INR) is essential
- Numerous factors may influence patient's response to anticoagulants
Overdose Effects
- Excessive depression of the coagulation activity can be corrected by temporary withdrawal of warfarin
- Vitamin K may be required to supplement specific treatment with co-factor concentrates
