  • Interrupts the coagulation cascade
  • Creates a hypercoagulable state for a short time

Dosage & Administration

  • Usual adult induction dose of warfarin is 10 mg daily for 2 days
  • Subsequent maintenance dose depends upon the prothrombin time (INR)
  • Maintenance dose is usually 3 to 9 mg daily
  • Initial dose for children is usually 0.1 mg.kg^-1.d^-1

Interaction

Has a greater potential for clinically significant drug interactions

Contraindications

Precautions & Warnings

  • Numerous factors may influence response to anticoagulants
  • Monitor patient's response with additional PT/INR determination after discharge from the hospital, and whenever other medications are initiated, discontinued or taken irregularly

Overdose Effects

Excessive depression of the coagulation activity can be corrected by temporary withdrawal of warfarin accompanied by infusion of fresh-frozen plasma or whole blood

Therapeutic Class
