Indications
- Treatment of mild, moderate or severe heart failure
- Treatment of ischemic or cardiomyopathic origin heart failure
- Reduction of disease progression
- Conjunction with digitalis, diuretics and ACE inhibitor
- Use in patients unable to tolerate an ACE inhibitor
Pharmacology
- Non-selective antagonism of β-adrenergic receptors
- α-adrenergic antagonistic activity
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Used in the treatment of hypertension
- Used in the treatment of angina pectoris
- Used in the treatment of congestive heart failure
Dosage & Administration
- Initial dose of 12.5 mg once daily for hypertension
- Increased dose to the usual dose of 25 mg once daily for hypertension
- Initial dose of 12.5 mg twice daily for angina pectoris
- Recommended dosage of 25 mg twice daily for angina pectoris
- Initial dose of 3.125 mg twice daily for heart failure
- Increased dose at intervals of at least 2 weeks for heart failure
Interaction
- Increased levels of digoxin when taken together with Koreg
- Recommendation for increased monitoring of digoxin
- 60% decrease in Cmax and AUC with rifampin pretreatment
- Did not alter the in vitro plasma protein binding of warfarin
- β-receptor antagonists potentiate the pressor reaction with clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker in place
- Cardiogenic shock
- Severe bradycardia

- Not to be used during breast-feeding
- No studies on safety and efficacy in children
- Not to be used during pregnancy
- Animal studies have shown crossing of the placental barrier
- No information on safety and efficacy in neonates
Precautions & Warnings
- Caution in hepatic impairment
- Monitor clinical status for 2-3 hours after initiation and dose increase
