Indications
- Reduction of total cholesterol and LDL cholesterol in familial hypercholesterolaemia

- Reduction of cardiac ischaemic events in coronary artery disease
- Reduction of cholesterol levels associated with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
Absorption
Rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours
Distribution
- Mean volume: Approximately 381 liters
- Plasma protein binding: 98%
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives
- Metabolized by cytochrome P450 3A4
- 70% of circulating inhibitory activity attributed to active metabolites
Excretion
- Primarily in bile following hepatic and/or extra-hepatic metabolism
- Mean plasma elimination half-life approximately 14 hours
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Changes in plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases

- Pediatric: Pharmacokinetic data not available
- Gender: No clinically significant difference in LDL-C reduction between men and women
- Renal insufficiency: No influence on plasma concentrations or LDL-C reduction
- Hemodialysis: Not expected to significantly enhance clearance
- Hepatic insufficiency: Markedly increased plasma concentrations in chronic alcoholic liver disease
