Indications
- Reduction of total cholesterol and LDL cholesterol in familial hypercholesterolaemia
- Treatment of mixed dyslipidemia (Fredrickson Type Ia and Ib)
- Treatment of hypertriglyceridaemia (Fredrickson Type IV)
- Treatment of dysbetalipoproteinaemia (Fredrickson Type III)
- Reduction of cardiac ischaemic events in patients with coronary artery disease
- Reduction of cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Rapid absorption after oral administration
- Extensively metabolized to active metabolites
DosageAdministration
- Adults: Usually 10 mg once daily; maximum 80 mg once daily
- Child (10-18 years): Initially 10 mg once daily, increased to max 20 mg once daily
- Dose adjustment based on lipid levels analysis
- Recommended to be taken on a standard cholesterol-lowering diet
- Dosing regimen analyzed within 2 to 4 weeks
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Plasma concentrations altered when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives
- No clinically significant effect on prothrombin time when administered with warfarin
Contraindications
- Hypersensitivity to any component
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to HMG-CoA reductase inhibitors

- Liver function tests performed before initiation of treatment and periodically thereafter
- Use with caution in patients with substantial alcohol consumption or history of liver disease
- Therapy discontinued if markedly elevated CPK levels occur or myopathy is diagnosed or suspected

- Elevated plasma concentrations in healthy elderly subjects
- No clinically significant difference in LDL-C reduction between men and women
- Renal disease has no influence on plasma concentrations or LDL-C reduction
