Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Inhibits cholesterol synthesis in the liver
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
Absorption
- Rapidly absorbed after oral administration
- Maximum plasma concentrations within 1 to 2 hours
- Extent of absorption increases with dose
Distribution
- Mean volume of distribution approximately 381 liters
- 98% bound to plasma proteins
- Likely to be secreted in human milk
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives
- Metabolism primarily by cytochrome P450 3A4
- Significant in vitro inhibition by metabolites
- Ortho-hydroxy metabolite undergoes further glucuronidation
Excretion
- Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
- Mean plasma elimination half-life approximately 14 hours
- Contribution of active metabolites extends half-life of inhibitory activity to 20 to 30 hours
Dosage & Administration
- Adults usually start with 10 mg once daily
- Dose may be increased at intervals of at least 4 weeks to max. 80 mg once daily
- Pediatric dose starts from 10 mg once daily
- Dose adjustment based on lipid levels within 2 to 4 weeks of initiation
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Plasma concentrations affected by coadministration with antacid, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

- Caution in patients with substantial alcohol consumption or history of liver disease

Use in Special Populations
- Higher plasma concentrations in elderly subjects
- No influence on plasma concentrations or LDL-C reduction in renal insufficiency
- Markedly increased plasma concentrations in chronic alcoholic liver disease
Overdose Effects
- No specific treatment available for atorvastatin overdose
- Symptomatic treatment and supportive measures required
- Liver function tests and serum CK levels monitoring recommended
