Pharmacology

  • Selective inhibitor of HMG-CoA reductase
  • Lowering plasma cholesterol and lipoprotein levels
  • Inhibiting cholesterol synthesis
  • Increasing hepatic LDL receptors
  • Rapid absorption after oral administration
  • Extensive metabolism by cytochrome P450 3A4
  • Elimination primarily in bile
  • Plasma elimination half-life of approximately 14 hours

Dosage & Administration

  • Adults dosage of 10-80 mg once daily
  • Child (10-18 years) dosage of 10-20 mg once daily
  • Starting dose of 10 or 20 mg once daily
  • Dosage range of 10 to 80 mg once daily
  • Lipid level analysis within 2 to 4 weeks of initiation

Interaction

  • Increased risk of myopathy with concurrent administration of certain drugs
  • Altered plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives
  • No clinically significant effect on prothrombin time when administered with warfarin

Contraindications

  • Hypersensitivity to any component
  • Active liver disease or unexplained persistent elevations of serum transaminases
  • History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

  • Higher plasma concentrations in elderly subjects
  • No clinically significant difference in LDL-C reduction between men and women
  • No influence on plasma concentrations or LDL-C reduction in renal disease
