Active ingredient
Atorvastatin
Mechanism of action
Atorvastatin is a selective inhibitor of HMG-CoA reductase, responsible for the conversion of HMG-CoA to mevalonate, a precursor of sterols including cholesterol. It lowers plasma cholesterol and lipoprotein levels by inhibiting cholesterol synthesis in the liver and increasing the number of hepatic LDL receptors for enhanced uptake and catabolism of LDL.
Absorption
Rapidly absorbed after oral administration with maximum plasma concentrations occurring within 1 to 2 hours.
Bioavailability
Approximately 14%
Distribution
- Mean volume: 381 liters
- Plasma protein binding: 98%
Metabolism
- Pathways:
- Cytochrome P450 3A4 pathway
- Inhibition potential: In vitro studies suggest the importance of Atorvastatin metabolism by cytochrome P450 3A4, consistent with increased plasma concentrations of Atorvastatin in humans following coadministration with erythromycin.
Elimination
- Excretion: Primarily in bile
- Plasma elimination half life: Approximately 14 hours
Indication
- Heterozygous Familial Hypercholesterolaemia
- Homozygous Familial Hypercholesterolaemia
- Hypertriglyceridaemia
- Cardiac Ischaemic Events
- Hypercholesterolemia Associated with Diabetes Mellitus
