Description

Liptor 40 mg Tablet is a selective inhibitor of HMG-CoA reductase. It is used to reduce total cholesterol, LDL cholesterol, and triglyceride levels. It is also indicated for the prevention of cardiovascular events and has contraindications in certain populations.

DosageAndAdministration

Usually 10 mg once daily for adults, may be increased to max. 80 mg once daily if necessary. For children (10-18 years), the initial dose is 10 mg once daily, increased if necessary to a usual max. of 20 mg once daily.

Pharmacology

  • ModeOfAction: Selective inhibitor of HMG-CoA reductase responsible for lowering plasma cholesterol and lipoprotein levels.
  • Absorption: Rapidly absorbed with maximum plasma concentrations occurring within 1 to 2 hours.
  • Distribution: Mean volume of distribution is approximately 381 liters, with 98% bound to plasma proteins.
  • Metabolism: Extensively metabolized to active metabolites, mainly by cytochrome P450 3A4.
  • Excretion: Primarily eliminated in bile following hepatic and/or extra-hepatic metabolism.

Interactions

  • Antacid: Plasma concentrations of atorvastatin decreased approximately 35% when coadministered with antacid suspension, but LDL-C reduction was not altered.
  • Colestipol: Plasma concentrations of atorvastatin decreased approximately 25% when coadministered with colestipol, but LDL-C reduction was greater.
  • Digoxin: Steady-state plasma digoxin concentrations increased by approximately 20% when coadministered with atorvastatin.
  • Erythromycin: Plasma concentrations of atorvastatin increased approximately 40% with coadministration of erythromycin.

Contraindications

Hypersensitivity to any component, active liver disease, unexplained elevations of serum transaminases, and history of serious adverse reactions to HMG-CoA reductase inhibitors.

  • Gender: No clinically significant difference in LDL-C reduction with Liptor between men and women.
  • RenalInsufficiency: No influence on plasma concentrations or LDL-C reduction; dose adjustment is not necessary.
  • Hemodialysis: Not expected to significantly enhance clearance of Liptor due to extensive plasma protein binding.
  • HepaticInsufficiency: Markedly increased plasma concentrations in patients with chronic alcoholic liver disease.

OverdoseEffects

Specific treatment is not available for atorvastatin overdose. Symptomatic and supportive measures should be instituted.
