Indications
- Management of hypertension and angina pectoris
- Treatment of cardiac arrhythmias
- Adjunct treatment of hyperthyroidism
- Reducing infarct size in acute myocardial infarction
- Pain relief and decreased need for opiate analgesics
- Reduction of mortality in acute myocardial infarction patients
Mode of Action
- Selective beta1-blocker
- Reduces or inhibits the agonistic effect on the heart of catecholamines
- Reduces heart rate, cardiac output, cardiac contractility, and blood pressure
- Interferes less with insulin release and carbohydrate metabolism
- Interferes much less with the cardiovascular response to hypoglycemia
Dosage & Administration
- Total daily dosage Metoprolol 100-400 mg for hypertension
- Metoprolol 50-100 mg for angina pectoris
- Maintenance dose of 200 mg daily for myocardial infarction
- Initial dosage of 25-100 mg for extended-release tablets
Interaction
- Additive effect with catecholamine‐depleting drugs
- Increased concentration with CYP2D6 inhibiting drugs
- Increased risk of bradycardia with concomitant use of digitalis glycosides
- Exacerbation of rebound hypertension when clonidine is withdrawn
Contraindications
- AV block
- Uncontrolled heart failure
- Severe bradycardia
- Sick-sinus syndrome
- Cardiogenic shock
- Severe peripheral arterial disease
- Known hypersensitivity to Metoprolol or other B-blockers

Pregnancy & Lactation
- Pregnancy Category C
- Excretion in breast milk in very small quantities
Precautions & Warnings
- Caution in patients with bronchospastic disease
- Controversy regarding withdrawing beta‐blocking therapy prior to major surgery
- Masking of tachycardia occurring with hypoglycemia
- Aggravation of symptoms of arterial insufficiency in peripheral vascular disease patients
Use in Special Populations
- Caution in patients with impaired hepatic function
- No clinically relevant differences in pediatric patients aged 6 to 16 years
- No notable differences in efficacy or adverse events between older and younger patients
Overdose Effects
- Severe hypotension
- Sinus bradycardia
- Atrioventricular block
- Cardiogenic shock
- Bronchospasm
- Impairment of consciousness
- Coma
- Nausea
- Vomiting
- Cyanosis
- Hypoglycemia
- Hyperkalemia
