Indications
- Management of hypertension and angina pectoris
- Treatment of cardiac arrhythmias
- Adjunct to the treatment of hyperthyroidism
- Early intervention in acute myocardial infarction
- Pain relief to decrease the need for opiate analgesics
- Reduction of mortality in acute myocardial infarction
Pharmacology
- Selective beta1-blocker
- Reduction or inhibition of the agonistic effect on the heart of catecholamines
- Reduction in heart rate, cardiac output, cardiac contractility, and blood pressure
- Less interference with insulin release and carbohydrate metabolism
- Less interference with the cardiovascular response to hypoglycemia
Dosage & Administration
- Dosage range for hypertension: 100-400 mg daily
- Starting dose for hypertension: 100 mg daily
- Individualized dosage for angina pectoris
- Dosage for cardiac arrhythmias control: 50-300 mg daily
- Dosage for hyperthyroidism: 50 mg four times a day
- Oral therapy for myocardial infarction: 50 mg every 6 hours for 48 hours
Interaction
- Additive effect with catecholamine‐depleting drugs
- Increased concentration with CYP2D6 inhibitors
- Risk of bradycardia with concomitant use of digitalis glycosides
- Exacerbation of rebound hypertension with withdrawal of clonidine
Contraindications
- AV block
- Uncontrolled heart failure
- Severe bradycardia
- Sick-sinus syndrome
- Cardiogenic shock
- Severe peripheral arterial disease

- Use with caution in patients with bronchospastic disease
- Controversy regarding withdrawal prior to major surgery
- Masking of tachycardia occurring with hypoglycemia
- Precipitation or aggravation of symptoms of arterial insufficiency
- Caution in patients treated with calcium channel blockers concomitantly
Use in Special Populations
- Caution in patients with impaired hepatic function
- No clinically relevant differences in adverse event profile for pediatric patients aged 6 to 16 years
- Safety and effectiveness not established in patients <6 years of age
- No notable differences in efficacy or rate of adverse events between older and younger patients
