Medicine Details

Indications

  • Lower respiratory tract infections
  • Upper respiratory tract infections
  • Otitis media
  • Skin and soft tissue infections
  • Sexually transmitted diseases in men and women

Pharmacology

  • Orally administered
  • Readily absorbed
  • Time to peak concentration: 2.1 to 3.2 hours
  • High concentration in phagocytes
  • Actively transported to the site of infection
  • Bacterial protein synthesis interference
  • Active against various microorganisms

Dosage

  • 500 mg once daily for 3 days
  • 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days
  • 1 gm single dose for Chlamydia trachomatis
  • 10 mg/kg body weight once daily for 3 days for child over 6 months
  • Azithromycin injection for IV infusion

Administration

  • Reconstitution procedure for suspension
  • Shake the bottle well to loosen the powder
  • Add boiled and cooled water
  • Take at least 1 hour before or 2 hours after meal

Interaction

  • Antacid interaction
  • Carbamazepine interaction
  • Cyclosporin interference
  • Digoxin metabolism impairment
  • Avoid co-administration with ergot derivatives
  • No significant effect on theophylline
  • No alteration of warfarin's anticoagulant effect

Contraindications

  • Hypersensitivity to Azithromycin or macrolide antibiotics
  • Co-administration with ergot derivatives
  • Patients with hepatic diseases

Side Effects

  • Mild to moderate gastrointestinal side effects
  • Allergic reactions such as rash or photosensitivity
  • Reversible elevations in liver transaminases
  • Rare cases of serious hypersensitivity reactions
  • Cases of cholestatic jaundice
  • Rare transient mild reductions in neutrophil counts
  • Reversible hearing impairment with prolonged high doses

  • Pregnancy Category B
  • Demonstrated no harm to the fetus in animal studies
  • Caution during pregnancy and lactation
  • No conclusive studies in pregnant women regarding safety

Precautions & Warnings

  • Rare serious allergic reactions reported
  • Caution in patients with renal impairment
  • Avoid use in patients with severe hepatic impairment

Use in Special Populations

  • No dose adjustment in mild renal impairment
  • Caution in patients with severe renal impairment
  • Avoid use in patients with hepatic disease
  • No evidence of effects on driving or machine operation

Overdose Effects

  • Typical symptoms include hearing loss, severe nausea, vomiting, and diarrhea
  • General supportive measures indicated
