Dosage and administration

  • Adult Dosage: Starting dose of 5 mg once daily, which may be increased to 10 mg and then 20 mg if necessary. Children: Safety and effectiveness not established. Patients with Renal or Hepatic Impairment: Initial daily dose should be 5 mg. Geriatrics: Dose adjustment not usually necessary unless significant renal or hepatic dysfunction.

Indications

  • Used in hypertension, angina, and moderate to severe heart failure.

Contraindications

  • Contraindicated in patients with cardiogenic shock, overt heart failure, second or third degree A-V block, right ventricular failure secondary to pulmonary hypertension, and sinus bradycardia.

Side effects

  • Possible side effects include fatigue, dizziness, headache, disturbances of the gut, cold or numb extremities, muscle weakness or cramps, breathing difficulties, and bronchospasm in people with asthma or COPD.

Pregnancy and lactation

  • Pregnancy: Should be used only if the potential benefit justifies the potential risk to the fetus. Lactation: If essential, mothers should stop nursing while using this medication.

Precautions and warnings

  • Caution in adjusting the dose for patients with impaired renal or hepatic function. Patients with a history of severe anaphylactic reaction may be more reactive to allergens and unresponsive to usual doses of epinephrine.

Interaction

  • Should not be combined with other β-blocking agents. Monitoring required when combined with catecholamine-depleting drugs, centrally active antihypertensive agents, antiarrhythmic agents, or calcium channel blockers.
