Indications

  • Hypertension
  • Angina
  • Moderate to severe heart failure

Pharmacology

  • High affinity for ß1 receptor
  • Reduces heart rate and cardiac output
  • Low inter and intra-individual variability
  • High absorption rate
  • Extensive distribution
  • Metabolized via oxidative pathways
  • Renally eliminated metabolites
  • Primarily metabolized via CYPSA4
  • Clearance balanced between renal elimination and hepatic metabolism
  • Elimination half-life of 10-12 hours

Dosage & Administration

  • Adult starting dose of 5 mg once daily
  • Potential dose titration to 20 mg once daily
  • Dosage individualized to patient's needs
  • Appropriate interval for dose titration is 2 weeks
  • No established safety and effectiveness in children
  • Caution needed in patients with renal or hepatic impairment
  • No need for dose adjustment in the elderly unless significant renal or hepatic dysfunction

Interaction

  • Not recommended to combine with other β-blocking agents
  • Monitoring needed when combined with catecholamine-depleting drugs
  • Caution with centrally active antihypertensive agents
  • Use with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
  • Combined use with calcium channel blockers may lead to severe hypotension, bradycardia, and cardiac failure

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension
  • Sinus bradycardia

Side Effects

  • Fatigue
  • Dizziness
  • Headache
  • Disturbances of the gut such as nausea, vomiting, diarrhea, constipation, or abdominal pain
  • Cold or numb extremities
  • Muscle weakness or cramps
  • Breathing difficulties
  • Bronchospasm in people with asthma or COPD

Pregnancy & Lactation

  • Potential teratogenic and fetotoxic effects in animal studies
  • No studies in pregnant women
  • Use during pregnancy only if potential benefit justifies potential risk to the fetus
  • Small amounts detected in the milk of lactating rats
  • Unknown excretion in human milk
  • Nursing mothers should consider discontinuing medication

Precautions & Warnings

  • Caution in adjusting the dose in patients with renal or hepatic impairment
  • Patients with history of severe anaphylactic reaction may be more reactive to allergens and unresponsive to usual doses of epinephrine
