Description
- Betapro is indicated for hypertension, angina, moderate to severe heart failure, and is not recommended for the emergency treatment of hypertensive crises.

Pharmacology
- Bisoprolol Hemifumarate is the most selective ß1 blocker
- Reduces heart rate and cardiac output resulting in decreased arterial hypertension
- Pharmacokinetic properties support a single daily dose and ensure low variability of plasma concentration profiles
Dosage and administration
- Starting dose for mild to moderate hypertension is 5 mg once daily, titration to 10 mg and then to 20 mg may be necessary
- Appropriate interval for dose titration is 2 weeks
- Safety and effectiveness in children have not been established
- Dosage adjustments needed for patients with renal or hepatic impairment
Interactions
- Not to be combined with other β-blocking agents
- Cautious use with catecholamine-depleting drugs and centrally active antihypertensive agents
- Careful use when coadministered with antiarrhythmic agents or calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia
Side effects
- Fatigue
- Dizziness
- Headache
- Disturbances of the gut
- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties
Pregnancy and lactation
- Bisoprolol fumarate should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
- Mothers should stop nursing if use of bisoprolol fumarate is considered essential
Precautions and warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with a history of severe anaphylactic reaction may be more reactive to repeated challenge
- Unresponsive to usual doses of epinephrine used to treat allergic reactions
