Pharmacology

  • High affinity for ß1 receptor
  • Reduces heart rate and cardiac output
  • Low inter and intra-individual variability in plasma concentration profiles

Dosage & Administration

  • Adult: 5 mg once daily, may be increased to 10 mg and then to 20 mg if necessary
  • Children: Safety and effectiveness not established
  • Renal or Hepatic Impairment: Initial daily dose of 5 mg
  • Geriatrics: Dose adjustment usually not necessary

Interactions

  • Other β-blocking Agents: Should not be combined with other β-blocking agents
  • Catecholamine-Depleting Drugs: Patients receiving catecholamine-depleting drugs should be monitored closely
  • Centrally Active Antihypertensive Agents: May exacerbate rebound hypertension
  • Antiarrhythmic Agents: Use with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
  • Calcium Channel Blockers: Combined use can lead to severe hypotension

Contraindications

  • Cardiogenic shock
  • Overt heart failure
  • Second or third degree A-V block
  • Right ventricular failure secondary to pulmonary hypertension
  • Sinus bradycardia
