Description
Beta-blocker medication for hypertension, angina, and heart failure
Dosage and administration
Adult starting dose 5 mg once daily, may increase to 10-20 mg. No established safety and effectiveness in children. Caution in patients with renal or hepatic impairment.
Pharmacology
- Mode of action: Selectively blocks ß1 adrenergic receptor in the heart and vascular smooth muscle
- Absorption and bioavailability: Bisoprolol is almost completely absorbed (>90%) from the gastrointestinal tract. Absolute bioavailability of 88%, unaffected by food intake.
- Metabolism: Metabolized via oxidative pathways with no subsequent conjugation. Primarily metabolized via CYPSA4 (-95%) with CYP2D6 having only a minor role.
- Elimination: Balanced between renal elimination and hepatic metabolism, clearance approximately 15 L/h, elimination half-life of 10-12 hours.
Interaction
- Other beta blocking agents: Should not be combined with other β-blocking agents.
- Catecholamine depleting drugs: Patients receiving catecholamine-depleting drugs should be monitored closely.
- Centrally active antihypertensive agents: β-blockers may exacerbate rebound hypertension following withdrawal of clonidine.
- Antiarrhythmic agents: Should be used with care when myocardial depressants or inhibitors of A-V conduction are used concurrently.
- Calcium channel blockers: Combined use with calcium channel blockers can lead to prolongation of S-A and A-V conduction.

- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia

- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties
- Bronchospasm
Pregnancy and lactation
- Pregnancy: Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
- Lactation: Not known if excreted in human milk. Mothers should stop nursing if use is considered essential.
Precautions and warnings
- Impaired renal or hepatic function
- Patients with history of severe anaphylactic reaction may be more reactive to allergens
