Dosage & Administration
- Usual starting dose of 5 mg once daily
- May be increased to 10 mg or 20 mg once daily

- Initial daily dose in patients with hepatic or renal impairment is 5 mg
- Dose adjustment caution in patients with impaired renal or hepatic function
- No need for dose adjustment in geriatric patients
Interaction
- Not recommended for combination with other β-blocking agents
- Monitor closely when combined with catecholamine-depleting drugs
- Caution when combined with centrally active antihypertensive agents
- Use with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
- Combined use with calcium channel blockers with negative inotropic effects can lead to severe hypotension
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia
Side Effects
- Fatigue
- Dizziness
- Headache
- Disturbances of the gut
- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties due to bronchospasm in people with asthma or COPD
Pregnancy & Lactation
- Not teratogenic in rats at doses up to 150 mg/kg/day
- Fetotoxic and maternotoxic effects observed in rats at higher doses
- No studies in pregnant women
- Small amounts excreted in the milk of lactating rats
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with severe anaphylactic reaction history may be more reactive to repeated challenge
- Unresponsive to usual doses of epinephrine used to treat allergic reactions
