Pharmacology

  • Selective beta1-blocker
  • Reduction/inhibition of agonistic effect on the heart of catecholamines
  • Reduction of heart rate, cardiac output, cardiac contractility, blood pressure
  • Interference with insulin release and carbohydrate metabolism
  • Less interference with cardiovascular response to hypoglycemia

Dosage & Administration

  • Total daily dosage range: 100-400 mg
  • Single or twice-daily dosage
  • Initial dosage: 100 mg per day, may be increased by 100 mg per week
  • Combination therapy with diuretic or other antihypertensive agents
  • Individualized dosage for angina pectoris and cardiac arrhythmias
  • Specific dosages for hypertension, angina pectoris, cardiac arrhythmias, hyperthyroidism, myocardial infarction
  • Dosage adjustment in elderly and patients with significant hepatic dysfunction
  • Extended-release tablet dosage guidelines

Interaction

  • Additive effect with catecholamine-depleting drugs
  • Increased concentration with CYP2D6 inhibiting drugs
  • Potential risk with concomitant use of digitalis glycosides
  • Exacerbation of rebound hypertension with clonidine withdrawal

Contraindications

  • AV block
  • Uncontrolled heart failure

  • Severe peripheral arterial disease
  • Hypersensitivity to Metoprolol or other B-blockers

Precautions & Warnings

  • Caution in bronchospastic diseases
  • Controversy over withdrawing beta‐blocking therapy prior to major surgery
  • Effect on tachycardia occurring with hypoglycemia
  • Precipitation or aggravation of symptoms of arterial insufficiency in peripheral vascular disease
  • Caution in patients treated concomitantly with calcium channel blockers

Use in Special Populations

  • Caution in patients with impaired hepatic function
  • Clinical profile in pediatric patients aged 6 to 16 years similar to adults
  • Not established safety and effectiveness in patients <6 years of age
  • Similar efficacy and adverse event rate in older and younger patients

Overdose Effects

  • Manifestations of hypotension, sinus bradycardia, atrioventricular block, heart failure, cardiogenic shock, bronchospasm, impairment of consciousness, coma, nausea, vomiting, cyanosis, hypoglycemia, hyperkalaemia
  • Treatment includes close monitoring of cardiovascular, respiratory, renal function, blood glucose, electrolytes, and symptomatic cardiovascular complication management
