Indications
- Management of hypertension and angina pectoris
- Treatment of cardiac arrhythmias
- Adjunct to the treatment of hyperthyroidism
- Early intervention in acute myocardial infarction
- Pain relief
- Reduction of mortality in acute myocardial infarction
Pharmacology
- Selective beta1-blocker
- Reduction/inhibition of agonistic effect on the heart of catecholamines
- Reduction in heart rate, cardiac output, contractility, and blood pressure
- Interference with insulin release and carbohydrate metabolism
- Interference with cardiovascular response to hypoglycemia
Dosage & Administration
- Total daily dosage of Metoprolol 100-400 mg for hypertension
- Initial dosage of 100 mg for hypertension
- Dosage range of 50-100 mg for angina pectoris
- 50 mg b.i.d or t.i.d for cardiac arrhythmias
- 50 mg four times a day for hyperthyroidism
- Oral therapy for myocardial infarction
- Usual initial dosage of 25-100 mg daily for extended-release tablet in hypertension
Interaction
- Additive effect with catecholamine-depleting drugs
- Increase in Selomet concentration with CYP2D6 inhibitors
- Increased risk of bradycardia with concomitant use of digitalis glycosides
- Exacerbation of rebound hypertension with beta-blockers withdrawal
Contraindications
- AV block
- Uncontrolled heart failure
- Severe bradycardia
- Sick-sinus syndrome
- Cardiogenic shock
- Severe peripheral arterial disease
- Known hypersensitivity to Metoprolol or other B-blockers
- Complication with significant bradycardia, first-degree heart block, systolic hypotension, and/or severe heart failure in myocardial infarction

Precautions & Warnings
- Caution in patients with bronchospastic disease
- Controversy regarding withdrawal of beta-blocking therapy prior to major surgery
- Masking of tachycardia in hypoglycemia by beta-blockers
- Precipitation or aggravation of symptoms of arterial insufficiency in peripheral vascular disease patients
- Caution in patients concomitantly treated with calcium channel blockers
Use in Special Populations
- Caution in patients with impaired hepatic function
- No clinically relevant differences in adverse event profile for pediatric patients
- Safety and effectiveness not established in patients <6 years of age
- No notable differences in efficacy or adverse events between older and younger patients
Overdose Effects
- Severe hypotension
- Sinus bradycardia
- Atrioventricular block
- Heart failure
- Cardiogenic shock
- Cardiac arrest
- Bronchospasm
- Impairment of consciousness
- Coma
- Nausea
- Vomiting
- Cyanosis
- Hypoglycemia
- Hyperkalemia
- Treatment including close monitoring of cardiovascular, respiratory and renal function
- Further absorption prevention by induction of vomiting, gastric lavage or administration of activated-charcoal
- Symptomatic treatment of cardiovascular complications
- Possible requirement for temporary pacing for AV block
- Reversal of excessive B-blockade effects with glucagon
- Requirement of intravenous B2-stimulants to relieve bronchospasm
- Ineffectiveness of haemodialysis for Selomet removal
