Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in elderly patients.
Pharmacology
- Beta adrenergic receptor blocking agent
- Inhibition of beta1 and beta1 adrenergic receptors
- Lack of intrinsic sympathomimetic and membrane stabilizing activity
- Decreased heart rate
- Decreased myocardial contractility
- Diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers
- Suppression of renin activity
- Vasodilation and decreased peripheral vascular resistance
- Metabolized by multiple routes including glucuronidation and hydroxylation by CYP2D6
- Mean peak plasma concentrations occur approximately 1.5 to 4 hours post-dosing
- Predominantly metabolized via direct glucuronidation of parent and to a lesser extent via N-dealkylation and oxidation via cytochrome P450 2D6
Dosage & Administration
- Individualized dosing based on patient's needs
- Recommended starting dose of 5 mg once daily
- Dose can be increased at 2-week intervals up to 40 mg for patients requiring further reduction in blood pressure
Interaction
- Use with care when myocardial depressants or inhibitors of AV conduction are used concurrently
- Avoid combined use with other beta-blockers
- Caution when co-administered with CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.)
Contraindications
- Severe bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome (without a permanent pacemaker in place)
- Severe hepatic impairment (Child-Pugh >B)
- Hypersensitivity to any component of this product
Side Effects
- Headache
- Nausea
- Bradycardia
Precautions & Warnings
- Abrupt cessation of therapy
- Caution in patients with coronary artery disease
- Cautious administration in compensated congestive heart failure
- Not studied in patients with angina pectoris or recent Ml
- Avoid use in patients with bronchospastic diseases
- Closely monitor patients when using anesthetic agents which depress myocardial function
- Caution in patients subject to spontaneous hypoglycemia or diabetic patients receiving insulin or oral hypoglycemic agents
- Monitoring for signs of hyperthyroidism
- Exercise caution in patients with peripheral vascular disease
- Caution in patients treated concomitantly with non-dihydropyridine calcium channel blockers
Use in Special Populations
- Caution in patients with severe renal impairment
- Contraindicated in patients with severe hepatic impairment
- Potential for increased reactivity to allergen challenges in patients with a history of severe anaphylactic reactions
