Indications
- Essential hypertension treatment
- Secondary hypertension treatment
- Treatment of left ventricular failure
- Treatment of Raynaud's phenomenon and Raynaud's disease
- Adjunct in the symptomatic treatment of urinary obstruction caused by benign prostatic hyperplasia
Pharmacology
- Decrease in total peripheral vascular resistance
- Selective inhibition of postsynaptic alpha-1-adrenoreceptors
- Lowering of blood pressure in supine and standing positions
- Reduction in left ventricular filling pressure
- Reduction in cardiac impedance
- Augmentation of cardiac output
- Improvement in urinary pressure profile in men with benign prostatic hyperplasia
- No adverse changes in the serum lipid profile
Dosage & Administration
- Initiation of therapy with a low starting dose
- Adjustment of dosage based on individual toleration
- Gradual dosage increase until desired effect or maximum dosage is reached
- Different dosing regimens based on patient's current antihypertensive therapy, renal function, and condition being treated
- Recommended starting dosage and usual daily maintenance dosage for each indication
Interaction
- Clinical experience showing no adverse drug interactions with cardiac glycosides, hypoglycemic agents, tranquilizers and sedatives, antiarrhythmic agents, analgesic, antipyretic and anti-inflammatory agents
Contraindications
- Known sensitivity to quinazolines
Side Effects
- Common reactions including dizziness, headache, drowsiness, lack of energy, weakness, palpitations, nausea, vomiting
- Additional reactions such as diarrhea, constipation, abdominal discomfort, liver function abnormalities, pancreatitis, edema, orthostatic hypotension, dyspnea, faintness, tachycardia, nervousness, vertigo, hallucinations, depression, rash, pruritus, alopecia, urinary frequency, impotence
Pregnancy & Lactation
- No teratogenic effects seen in animal testing
- Use during pregnancy only if the potential benefit justifies the potential risk to the mother and fetus
- Excretion in small amounts in human milk
Precautions & Warnings
- Postural hypotension precautions for patients with hypertension
- Careful adjustment of diuretic dosage for patients with left ventricular failure
- Close monitoring of blood pressure for patients with Raynaud's Phenomenon/Disease and benign prostatic hyperplasia
- Not recommended for children under 12 years
Use in Special Populations
- Not recommended for children under 12 years
- Not recommended for left ventricular failure due to mechanical obstruction or recent myocardial infarction
Overdose Effects
- Support of the cardiovascular system in case of hypotension
- Monitoring and support of renal function
- Non-dialyzable because it is protein bound
