Indications
- Treatment of infections caused by sensitive Gram-positive and Gram-negative bacteria
- Upper respiratory tract infections like sinusitis, pharyngitis, otitis media
- Lower respiratory tract infections like bronchitis, pneumonia
- Urinary tract infections like cystitis, pyelonephritis
- Skin and soft tissue infections like cellulitis, abscess
- Susceptible microorganisms: Staphylococci, Streptococci, Escherichia coli, Klebsiella spp
- Effective against penicillin-resistant strains and penicillinase-producing species
Pharmacology
- Broad spectrum bactericidal antibiotic
- Active against gram-positive and gram-negative microorganisms
- Main site of action is the cell wall of bacteria, inhibits cross-linking process
Dosage & Administration
- Adult oral dosage: 250mg-1g depending on the site and severity of infection
- Child oral dosage: 25-50mg/kg in divided doses
- Injectable dosage: 2-4g daily in four equally divided doses
- Maximum daily dosage: 4g
- Dosage adjustment necessary in renal impairment
Interaction
- Concomitant use with nephrotoxic drugs may increase risk of kidney damage
- Diuretics and probenecid enhance possibility of renal toxicity
Contraindications
- Should not be used in patients with known or suspected hypersensitivity to cephalosporins
Side Effects
- Limited to gastrointestinal disturbances and hypersensitivity phenomena
- Possible skin reactions, glossitis, heartburn, dizziness, nausea
Pregnancy & Lactation
- Safety in pregnancy not established
- Excreted in breast milk, caution in lactating mothers
Precautions & Warnings
- Prolonged use may result in development of superinfection
- Administer with care to patients hypersensitive to penicillins
- May cause false positive urine glucose result with certain tests
Use in Special Populations
- Recommended doses for renal impairment and haemodialysis patients
Overdose Effects
- Non-specific symptoms like nausea, vomiting, diarrhoea
