Indications
- Mild-to-moderate hyprtension
- Severe hypertension
- Congestive heart failure
Pharmacology
- Competitive inhibition of angiotensin converting enzyme (ACE)
- Decreased levels of angiotensin II
- Increased plasma renin activity
- Decreased aldosterone secretion
- Rapid absorption from the GI tract
- Renal excretion of drug and metabolites
Dosage & Administration
- Initial dose for mild-to-moderate hypertension: 12.5 mg twice daily
- Usual maintenance dose for mild-to-moderate hypertension: 25mg twice daily
- Maximum dose for mild-to-moderate hypertension: 50 mg twice daily
- Starting dose for severe hypertension: 12.5 mg b.d.
- Maximum dose for severe hypertension: 50 mg t.d.s.
- Starting dose for heart failure: 6.25mg or 12.5mg
- Usual maintenance dose for heart failure: 25mg three times a day
- Maximum dose for heart failure: 150 mg daily
- Administration 1 hour before meals
- No established safety and effectiveness in pediatric patients
Interaction
- Coadministration with aliskiren in patients with diabetes is contraindicated
- Periodic monitoring of renal function
Contraindications
- History of hypersensitivity to captopril
- Contraindicated in pregnancy
- Avoid use in women of child-bearing potential without effective contraception
