Indications
- Mild-to-moderate hypertension as adjunct to thiazide therapy
- Severe hypertension when standard therapy has failed
- Congestive heart failure in combination with diuretics and digitalis
Pharmacology
- Sulfhydryl-containing analog of proline
- Competitively inhibits angiotensin converting enzyme (ACE)
- Decreases levels of angiotensin II
- Increases plasma renin activity
- Decreases aldosterone secretion
- Rapid absorption from the GI tract
- Excretion mostly renally
Dosage & Administration
- Initial dose for mild-to-moderate hypertension: 12.5mg twice daily
- Usual maintenance dose for mild-to-moderate hypertension: 25mg twice daily
- Maximum dose for mild-to-moderate hypertension: 50mg twice daily
- Starting dose for severe hypertension: 12.5mg twice daily
- Maximum dose for severe hypertension: 50mg three times daily
- Starting dose for heart failure: 6.25mg or 12.5mg
- Usual maintenance dose for heart failure: 25mg three times a day
- Maximum dose for heart failure: 150mg daily
- Must be taken 1 hour before meals for maximum absorption
- Not recommended for pediatric use
Interaction
- Do not coadminister aliskiren with Cardopril in patients with diabetes
- Monitor renal function periodically
Contraindications
- Hypersensitivity to captopril
- Contraindicated in pregnancy & women of child-bearing potential
