Indications
- Treatment of adult patients with active psoriatic arthritis
- Treatment of moderate to severe plaque psoriasis
- Candidates for phototherapy or systemic therapy
Pharmacology
- Orally available small molecule inhibitor of type-4 cyclic nucleotide phosphodiesterase (PDE-4)
- Increases intracellular levels of cAMP
- Inhibits production of multiple proinflammatory mediators
Dosage & Administration
- Recommended initial dosage titration from Day 1 to Day 5
- Recommended maintenance dosage of 30 mg twice daily starting on Day 6
- Can be administered without regard to meals
- Dosage adjustment in patients with severe renal impairment
Interaction
- Co-administration with strong cytochrome P450 enzyme inducers not recommended
Contraindications
- Contraindicated in patients with known hypersensitivity to Apremilast or any of the excipients in the formulation
Side Effects
- Frequently occurring side effects: nausea, diarrhea, headache
- Less frequent side effects: upper respiratory tract infection, vomiting, naospharyngitis, abdominal pain, hypersensitivity, gastroesophageal reflux disease, dyspepsia, fatigue, decrease appetite, cough, rash, insomnia
Pregnancy & Lactation
- Pregnancy Category C
- Caution advised when administered to a nursing woman
Precautions & Warnings
- Associated with an increase in adverse reactions of depression
- Patients should be advised to be alert for mood changes
- Evaluation of risks and benefits if depression or mood changes occur
- Weight decrease reported in some subjects
Use in Special Populations
- Safety and effectiveness not established in paediatric patients less than 18 years of age
